Requirements for Training

4 photocopies of Birth Certificate (NSO) copy

4 photocopies of Marriage Contract (FOR MARRIED WOMAN ONLY)

4 photocopies of High School Diploma/ Transcript of records

4 pcs. of 1x1 ID pictures

6 pcs. of Passport sized pictures(3.5 cm. w/ head size) forehead to chin (ranging from 27mm to 31mm) colored w/ white background, shirt/dress with collar,not scanned,not edited,not rush, no stapled damage and paper should not be glossy.
